The Cleveland Indians have activated Carlos Santana from their seven-day concussion list, according to Paul Hoynes of The Plain Dealer. In a corresponding roster move, the Indians have optioned shortstop Juan Diaz.

Santana last appeared in a game for the Indians on May 25 when he was struck in the catcher's mask by a foul tip. Santana was later evaluated and eventually placed on the seven-day concussion list.
Before rejoining the Indians, Santana spent one game in the minors rehabbing. Santana was the designated hitter for Cleveland’s Low-A affiliate last night, going 1-4 with a home run and two RBIs. On the season, Santana has hit at a clip of .245/.360/.387 with 12 extra-base hits and 24 RBIs.
Diaz had appeared in five games for the Indians this season. Over his 17 plate appearances, Diaz collected four hits and a walk. Diaz has spent a majority of the 2012 season playing for the Indians Double-A affiliate, where he’s maintained an OPS of .628.
